By submitting you interest and/or attending an interview you understand, acknowledge and/or agree to the following statements.
 
I understand the Blues have sole discretion to extend the selection process at any point. 
 
By attending an interview, you authorize the Blues Coach Selection Committee and its representatives to contact your references or others to verify the information contained herein, attached and disclosed during your interview, not limited to previous coaching staff and others who may be familiar with previous experience/background related to the position.
 
I, the applicant, agree that the decisions of the Blues Coach Selection Committee and the Blues (BMHA/EMHA) are final and I hold the Committee, the Blues (BMHA/EMHA), and any other governing party harmless for the decision(s). 
 
Initial interviews will be conducted between April 10-18 and maybe be extended at the discretion of the presidents based on the number of submissions.  Please note, candidates will receive their interview date/time by April 8.  It is the responsibility of the candidates to be available for their assigned date/time.

Topics of discussions may include but not limited to, coaching style/philosophy, season plans (past, present, future), coaching experiences, challenges.

2. Level interested in Coaching
3. Level Child played during 2025/2026 (If parent coach)
4. Have you applied for any other hockey coaching positions for the 2026-27 season?
5. Indicate the number of years you have been a head coach.
6. Indicate the number of years you have been an assistant coach.
7. Indicate the number of years you have been an on-ice helper.
8. If applicable, list the team(s) you have been a head coach or assistant coach (year, level, position, team/association)
9. As a coach or bench staff member, have you ever been suspended?  If yes, please provide the following details of the suspension(s) starting with the most recent: Date/Season, Infraction, Suspension Duration
